What OASIS Can Do for You and your Patients

OASIS is a referral and assessment program that aims to improve the quality of life and health outcomes for people with osteoarthritis by providing support, education, and information. Primary care physicians, rheumatologists, and surgeons helped develop the program and continue to be involved. 

Benefits for Patients

Our multidisciplinary team – made up of physiotherapists, occupational therapists, dietitians, and nurse clinicians – will assess your patient’s unique needs and work with both you and your patient to develop a personalized Action Plan. OASIS will help your patient navigate the health care system and access the services and resources they need to manage their condition.  Prioritization will be based on patient need.


Benefits for Physicians

OASIS assumes many of the administrative, education, and information tasks necessary to support people with osteoarthritis so physicians can focus on client care. OASIS screens patients before they are assessed by a surgeon and provides preoperative education and clerical support. Surgeons will find they spend less time completing forms and more time assessing appropriate surgical candidates.


Referring to OASIS

Primary care physicians, rheumatologists, and surgeons can refer their patients to OASIS for a comprehensive multidisciplinary assessment. To refer a patient, complete the OASIS Physician Referral Form (PDF) and fax the form to the OASIS Clinic your patient would like to attend. Be sure to include information about where we can view your patient’s most recent x-ray.

Physicians can also send information only patients to OASIS. These are usually people who are at the very early stages of the disease and want information about osteoarthritis and self-management. You may refer them to OASIS for information only or direct them to the OASIS website.
