
Multidisciplinary Assessment Appointments
You will be seen by two or more members of the OASIS multidisciplinary team that includes nurse clinicians, physiotherapists, occupational therapists, and dietitians. They will assess your condition and identify your needs.
OASIS will help ensure that you have timely access to our services by basing access on need.
For you to attend the program your family doctor, surgeon, or rheumatologist will have to fill out an OASIS Physician Referral Form (PDF).
Your Action Plan will be based on the findings of your assessment. It will outline recommendations for treatment, follow up, your personal self-management goals, and referrals to support services and community programs.
Access to Support Services and Treatment
OASIS will work with you and your family doctor to carry out the recommendations of your Action Plan. While you may need a referral from your doctor to access a specific service, many of the programs recommended by OASIS are open to the public.

If you are a candidate for surgery OASIS will assist your family doctor in referring you to a surgeon with a notation about your symptoms and level of surgical need.
OASIS offers a variety of free education sessions to enable you to self-manage your osteoarthritis at all stages of the disease.
